Purpose Physical Therapy is a progressive outpatient rehabilitation practice serving our community through a hybrid model of care. We provide evidence-based therapy in-clinic and through community/home-based services. Our diverse programs include outpatient orthopedics, neurology, women鈥檚 health, balance and fall prevention, home safety assessments, wheelchair evaluations, wound/ lymphedema care, and wellness programs. POSITION SUMMARY:

This role is responsible for providing medically prescribed, goal-oriented physical therapy in a patient鈥檚 home or select location based on treatment/patient needs. The role includes providing therapeutic treatments and participating in the development of patient physical therapy plans of care.

PRINCIPAL ACCOUNTABILITIES:

路 Perform initial and ongoing physical therapy evaluations and re-evaluations according to APTA standards of practice as well as state standards.

路 Develop a plan of care/treatment plan for each patient based on the initial evaluation and objective findings, including short and long-term goals, keeping the patient and caregiver as the focus of care.

路 Implement treatment plan on an established schedule and adjust this schedule and plan according to patient progress.

路 Document patient progress according to approved medical documentation and facility documentation procedures.

路 Evaluate mobility, strength, and current level of patient function, while setting goals to improve in these areas that are scaled to patient abilities and care realistically attainable.

路 Report patient progress to the patient, patient caregivers and healthcare professionals involved in the ongoing treatment/care of patients.

路 Develop a plan for patients following completion of their Plan of Treatment with appropriate support and follow-up services if needed.

路 Maintain professional and technical knowledge through attending educational workshops, reviewing professional publications, etc.

QUALIFICATIONS:

路 Compassionate and able to deliver quality patient-centered care

路 Excellent problem solving, interpersonal, and communication skills

路 Motivated and able be excel in a self-managed environment

路 Desire to work a flexible schedule based on patient needs

路 Ability to travel to patient homes or the desired location for treatment

EDUCATION:

路 Bachelor鈥檚 Degree in Physical Therapy from an accredited physical therapy program.

CERTIFICATIONS, LICENSES AND REGISTRATIONS:

路 Currently-licensed Physical Therapist in the state of Wyoming

路 Possesses and maintains a current CPR certification

路 Valid driver鈥檚 license and auto liability insurance
